Care Navigation Team

We work with people, aged 18 and over, to prevent them from being admitted to hospital unnecessarily.

We work closely with other services at Queen Mary’s Hospital and across Bexley to find the most appropriate treatment option for you. This may include referring you to an intermediate care unit, such as Step Up Step Down, or arranging treatment for you in your own home or in the community.

Access to the service is usually by referral from the Urgent Care Centre, but may also be through other services at the hospital.
